How to apply
Email your application to [email protected]
Include the following information:
- A covering letter – with the title ‘Application to reinstate parallel trade permit for [product name] in GB’; explaining that you are submitting an application to reinstate your previous parallel trade permit and giving details of that permit
- A copy of the previous CRD5 form (Word document)
- Copies of any later application to amend the permit before the end of December 2022
- Notification of any changes to the contact details, such as the applicant and permit holder address - you must detail these in the covering letter
- Confirmation that the product to be imported remains authorised and available for sale in the Member State of origin at the time of application and the product details on the label, such as product name, registration number, company name are the same as in the original parallel trade product application
- Photographs of the source product container with visible and readable label (including all sides of the container), showing the product as it is being marketed in the Member State of origin at the time of application for the reinstatement of the permit
- Confirmation that the draft label for the parallel product as submitted with the previous application remains unchanged, except where the GB reference product label has changed - then these changes must be reflected in the parallel trade product label, read the next point
- Where the label has changed, to reflect changes in the reference product label, then the changes must be notified together with a copy of the draft label to be marketed in GB which has all the changes highlighted
- A declaration that all the required information, as listed above, supplied is accurate and that the duplicate information continues to apply in respect of that product; this should be included in the covering letter
