How to report an ESQCR incident
Only duty holders under ESQCR should submit reports using this system.
If you are an employee (or representative) or a member of the public wishing to report an incident about which you have concerns, please refer to our advice.
Online
You can report your incident via the online form:
The online reporting system provides for four types of report:
- Injury (including fatalities)
- Other (including fires and explosions and low overhead lines)
- Supply interruptions
- Domestic fatalities
By telephone
All incidents can be reported online but a telephone service is also provided for reporting fatal and major injuries only. Call the Incident Contact Centre on 0345 300 9923 (opening hours Monday to Friday 8:30 am to 5.00pm). Outside of these hours, guidance is listed on theĀ ways to contact HSE webpage.
If you want to report less serious incidents out of normal working hours, you can always complete the ESQCR online report form.

Out of hours
The HSE and local authority enforcement officers are not an emergency service.
Information on the type of circumstances were HSE may need to respond out of hours are available on the ways to contact HSE page.
Alternatively, if you want to report less serious incidents outside of normal working hours, you can always complete the ESQCR online report form.
